General Questionnaire
The questionnaire is a mixture of multiple choice and 'open' questions.
The questionnaire will download to your computer so you do not need to be online while completing the questionnaire. The download may take a minute or two if you have a slow Internet connection.
There is no time limit for completing the questionnaire online but your views are not sent to us until you connect to the Internet and click on the Submit button on the last page. A series of dots are displayed at the bottom of each page to indicate your progress through the questions.
Please allow sufficient time to complete the whole questionnaire, as there is no option to save an incomplete questionnaire for completion at a later date. It is recommended you allow between 15 to 30 minutes to complete the questions.
Please answer as many questions as possible but ignore any which do not apply to you.
